Nurse‑Led Clinical Care at Home (TDDI)

Treatment of Disease, Disorder or Injury (TDDI)

Rockingham Home Care is CQC registered to provide Treatment of Disease, Disorder or Injury (TDDI), enabling us to deliver safe, structured, nurse‑led clinical care to people with complex health needs in their own homes.

This registration allows us to support individuals who require ongoing clinical input alongside personal care, helping them remain at home, avoid unnecessary hospital admissions, and maintain dignity, independence, and quality of life.

Our Clinical Approach

All clinical care delivered by Rockingham Home Care is nurse‑led and clinically governed. The service is led by our Registered Manager, who is also a Registered Nurse and Clinical Lead, providing clear accountability, professional oversight, and consistent decision‑making across the service.

Clinical care is delivered in line with:

  • Individualised care plans

  • Evidence‑based clinical protocols

  • Robust risk assessments

  • Ongoing monitoring and review

Healthcare tasks are only delegated to care staff where they have received appropriate training, been assessed as competent, and are working under clear nurse supervision.

Conditions We Support

We support adults with a wide range of diseases, disorders, and injuries, including:

  • Spinal injuries

  • Stroke

  • Parkinson’s disease

  • Huntington’s disease

  • Cerebral palsy

  • Acquired brain injuries

  • Other long‑term, progressive, or complex health conditions

Many of these conditions require skilled clinical input, careful monitoring, and a personalised approach to care.

Our nurse‑led service is able to provide and oversee a range of clinical treatments at home, including:

  • Medication management and clinical oversight

Strong Clinical Governance

At Rockingham Home Care, the Registered Manager also undertakes the role of Clinical Lead, ensuring strong clinical governance in line with CQC Regulation 17 (Good Governance).

This includes:

  • Clinical supervision and support

  • Staff competency assessment

  • Safe delegation of healthcare tasks

  • Clinical audits and quality monitoring

  • Incident management and learning

  • Risk management and escalation

As the service grows, we have clear plans to strengthen our team by recruiting additional registered nursing support and administrative capacity, ensuring resilience, continuity, and high standards of care.
